Really appreciated the tables summarizing information- easy to retrieve for future reference. As a pediatric aud trying to help fit her elderly father (and field inquiries from many of his older friends who are seeking satisfaction from their hearing aids!), I found this information extremely helpful. This also made me think more about the special needs population I work with (pediatric, yet with global processing difficulties) and how programming strategies are not one size fits all.

After much discussion of the complexities of the auditory system and the possible pitfalls of FAC, high CRs, etc, it was apparent that audiologists don’t have the information needed from most manufacturers to change the compression. I already knew to avoid high CRs. It is still a matter of trial and error.

I realized that there is cognitive decline and knew SAC is more beneficial with greater cognitive decline, but never completely comprehended all the cumulative factors to consider when adjusting hearing aids. This is information critical for audiologists fitting hearing aids. If we want to differentiate ourselves and encourage hearing impaired patients to seek professional care, we must know this information and deploy techniques to optimize hearing, otherwise, OTC hearing aids will for all intent and purpose will be the same as what we are fitting. It is also a call to manufacturers to publish technical data of these features AND allow us as professionals to have access to change.

This is the best article about hearing aid fitting I've seen, based both on research and priorizing clinical decision-making and responsiveness to client needs. Dr. Windle makes a number of points that are too often are ignored: current hearing aids are black boxes and we clinicians do not really understand how they work, manufacturer's do not report important hearing aid specifications, listening to patients and validating the fitting are most important, at least as important as verification.
