Option 1 – Use the CaptionCall Module for noah 4. Simply visit www.captioncall.com and download the module from the Professionals page.

Option 2 – Use the CaptionCall website. Simply visit www.captioncall.com and click the CERTIFY PATIENT tab where you can fill out and submit the online form.

Option 3 – Complete a hard copy or electronic copy of the Professional Certification Form and submit it following the instructions at the top of the form.

Once we receive the completed certification form, we will contact your certified patient and arrange for the free delivery and installation of the CaptionCall phone.
