More than 30 million U.S. adults have disabling hearing loss. In addition to negatively affecting the quality of life, hearing impairment is associated with dementia, depression, and social isolation. Despite the well-demonstrated benefits of hearing aids, relatively few eligible people (an estimated 15% to 30%) wear them. As federal regulations of over-the-counter hearing aids get closer to becoming reality, this course attempts to review the opportunities, threats, and challenges associated with hearing aids being sold over-the-counter without the direct involvement of a licensed audiologist. A primary focus of this course will be to demonstrate how the hearing care professional’s ability to customize along two fronts, customizing a start-gain target and customizing ear coupling, will allow them to stand apart from OTC and other online retailers.

Most audiologists would agree the professional landscape is changing. Over-the-counter and self-fitting hearing aids are on the horizon, reimbursement options, including the possibility Medicare will pay for hearing aids is looming in Washington DC, and telecare is fast becoming a routine part of clinical practice.
This changing landscape is spurring renewed interest in clinical standards, guidelines, and protocols. For example, the Audiology Practice Standards Organization (APSO) recently published clinical standards for fitting adults with hearing aids (see, https://www.audiologyonline.com/articles/20q-new-hearing-aid-fitting-27938). While standards tell us “what” we should be doing in the clinic to optimize patient outcomes, guidelines and protocols tell us “how” to conduct those best-practice standards.

This article reviews survey data from hearing aid wearers representing three different countries. Results of the survey provides evidence of improved wearer outcomes compared to their previous devices. The authors provide commentary on the performance enhancements in real world listening associated with the Signia AX split processing technology.

In the everyday practice of fitting hearing aids, we develop many beliefs, about both the technology and the fitting procedures. Some of these beliefs are based on things we think we have read, things we have heard from a rep, things from our fitting experiences, or things we just believe to be true because we always have. But for most of us, not all of them really are true. This presentation will reveal some of the most common misconceptions related to the fitting and operation of hearing aids.
