AIT and Progressive Hearing Loss

Question

I have a 16 yr old patient who has been receiving AIT for treatment of CAPD for the last 18 mos. (her parents fly her to California for this several times a year). I see her only for pre- and post- treatment audiograms. When she started with the AIT, her thresholds were all in the -5 to 10 dB range. Subsequent audiograms have shown a gradual decline in thresholds, to the point where she now has a slight SNHL. She now shows thresholds at 25 - 30 dB, particularly at 1000, 1500, 6000 and 8000Hz (with 3-4000 Hz rebounding to 0 - 5 dB). I am wondering if this decrease in thresholds can actually be due to AIT or if it is a coincidental progressive loss? Have detrimental effects been shown for AIT (other than wasting a lot of money if it doesn't work)?

Answer

I am unaware of any reports in the literature to implicate AIT and acquired hearing loss. In order to know about this specific case it would be necessary to have the details of the treatment. Nevertheless, until more is understood about this progressive hearing loss I would be extremely cautious in continuing, and would want complete otologic and audiologic testing to determine what is going on.
